3/31/2023 EnchantaVaughn Approved Behavior: During today’s visit, client appeared to be in a shifted mood, as evidenced by her irritable emotions and puzzled facial expressions. The client described a pattern of consistent misfortunate events that happened today. The client expresses feelings of bad luck due to certain things not going her way. Client verbalizes different complaints but utilizes her coping skills properly once she was in a more stable mindset. The client was supported for demonstrating a decrease in level of paranoia. Behavior: Client demonstrates ability to hold herself accountable and adapt to new challenges. Client was reminded to utilize relaxation techniques and breathe so that she could stay calm and focus in the session. Client expressed needing more help and agreed to be less emotional and more logical with handling stressful situations at home during this time. Client was appreciative of the therapeutic services and having someone to depend on that understood her on serious and personal level. Intervention: QMHP addressed client’s negative behavior right away. QMHP allowed client to individually process what occurred throughout the day. QMHP validated client’s feelings but explained the importance of utilizing her coping skills. QMHP stated “You have to learn to ignore others and not be so easily triggered.” QMHP isolated client from distractions to decrease disruptive behaviors and to reduce excessive talking. QMHP explored the client’s capability to cope with her stressors on her own. Intervention: QMHP verbally identified the client's impulsiveness and encouraged the client to use self-control strategies. QMHP brainstormed strategies with client and encourage her to implement them on a daily basis. QMHP also provided mediation and relaxation techniques to maintain appropriate behaviors. QMHP explained the importance of being on task and refraining from outside distractions. QMHP reviewed and gave feedback regarding ADHD and its causes and affect toward client well-being and academic functioning. Response: Client actively participated with QMHP as they discussed and processed her feelings. Client communicated that it is often hard to remain positive at all times about herself when she doesn’t always have the best things or necessities that she needs. Client discussed numerous situations where she had to adapt even when she didn’t want to but was thankful for the changes. Client was receptive as QMHP demonstrated better ways to function in the home setting. Response: Client displayed active behaviors in the second half of the session. Client engaged in self-reflection, discussing good and bad parts of her day. Client chose to get herself together and participate. Client communicated that she feels that she will be more successful, due to having a woman as a counselor. Mom expressed to the counselor that though client’s behavior has improved she still has to remind client about remaining consistent with her reactions and perceptions.
